Sports Medicine & Orthopedic Surgery P.C. now uses the online form filling feature found in Adobe Acrobat PDF (portable document format) files. Although one can simply print out any given form as is and then fill it out by hand, we encourage users to fill out the form using their computer so the form will be easier to read when processed.

It is important to review and print
BOTH of the following PDF files.

	Patient Welcome This PDF contains general patient information for you to print, review and sign.
	Patient Information This PDF contains medical questionnaires and forms for you to either fill out online and then print or print and fill out by hand.

1. Click on the Adobe Acrobat form you wish to use. The form will then load and appear within your browser's window.
   
   (If the form does not load, you may not have the Adobe Acrobat software available on your computer.)
   
   
2. Next, click inside any field in which you wish to enter information. If you place your mouse cursor over a field where information can be typed in, your cursor will change to a vertical line.
   
   When the cursor changes to a vertical line, simply click your left mouse key and begin entering the necessary information.
   
   
3. Enter the information you want to place into the form.
   You can use the "tab" key to jump from one field to the next.

5. Once you have filled out the form and printed it, all you need to do is sign the form (if required) and bring it with you to your appointment.
